Wiltshire Council has announced that Buckland Court will shut as part of wider changes to care provision across the county, aimed at modernising services and improving facilities.
The home, which cares for elderly residents including those living with dementia, is currently run by The Orders of St John Care Trust.
The organisation will work with the council to support residents as they are moved to alternative accommodation.
Relatives say the news has come as a shock, with many raising concerns about the impact the move could have on vulnerable residents - particularly those with dementia, who can find changes in environment especially distressing.
Wiltshire Council says the decision is part of a long-term plan, noting that some care home buildings are ageing and no longer suited to increasingly complex care needs.
Officials have stressed that residents will be supported throughout the process, with individual plans put in place to help ensure continuity of care and minimise disruption.
Care will continue as normal in the short term while arrangements are made, with the council pledging to work closely with families, residents and staff during the transition.
